MUSIC VIDEO RELEASE FORM

I, the undersigned, hereby grant Rockefellas Enterprises DBA; Rockefellas TV (hereinafter referred to as "Rockefellas TV"), the authorization to use, broadcast, publish, and distribute the music video submitted on our Roku channel (Rockefellas TV).

I hereby represent and warrant that I possess the necessary authority to grant this authorization on behalf of myself and all other parties who have rights in the Work.

I acknowledge and agree that Rockefellas TV may use and distribute the Work in any and all media, whether now known or hereafter developed, for any purpose, including but not limited to advertising and promotion of Rockefellas Enterprises DBA; Rockefellas TV and the Work.

By signing this release, I release Rockefellas TV, its officers, directors, employees, agents, representatives, successors, and assigns from any and all claims, demands, liabilities, damages, costs, and expenses, including reasonable attorneys' fees, arising out of or in connection with the use of the Work.

I understand and acknowledge that I will not receive any compensation for the use of the Work, and Rockefellas TV is under no obligation to use the Work.

I agree to provide Rockefellas TV with the following information pertaining to the Work:

• Song details, including title, composer, and publisher information

• Credits for the music video, including director, producer, and any other relevant crew members

• The International Standard Recording Code (ISRC) for the musical work

• The International Standard Music Video Code (ISMVC) for the video

• Video thumbnails

• Any other pertinent information related to the Work ​​​​

Furthermore, I represent and warrant that the Work does not infringe upon any third-party rights, including but not limited to copyright, trademark, or privacy rights.
This release shall be binding upon and inure to the benefit of the parties hereto and their respective heirs, legal representatives, successors, and assigns.

I hereby confirm that I have read this release and fully understand its contents. I am of legal age, being 18 years or older, and possess the right to enter into this agreement.
